Behavioral Health Care Manager
About The Position
Under the direction of the ECM Program Director, the Behavioral Health Care Manager provides comprehensive, member-centered care management services to individuals enrolled in California’s Enhanced Care Management (ECM) program. This role integrates behavioral health expertise with whole-person care coordination, including outreach, engagement, assessment, care planning, and coordination with healthcare and social service providers. The position operates within a Community-Based Care Management Entity (CB-CME) and collaborates across interdisciplinary teams to improve health outcomes, address social determinants of health, and ensure timely access to appropriate services.
Requirements
- Master’s degree in Social Work, Marriage & Family Therapy, Counseling, Psychology, or related field.
- Three (3) years of experience in behavioral health services within a healthcare clinic, psychiatric hospital medical facility, or community-based setting.
- Valid California Driver’s License and current automobile insurance when using a personal vehicle for Company business.
- Strong interpersonal and relationship-building skills.
- Proficient in evidence-based communication (e.g., Motivational Interviewing).
- Effective conflict resolution and professional communication skills.
- Ability to engage members and providers across in-person and telephonic settings.
- Cultural competence and sensitivity to diverse populations.
- Knowledge of mental health and substance use conditions across the care continuum.
- Ability to educate, coach, and provide clinical guidance to others.
- Awareness of implicit bias and commitment to equitable care.
- Ability to work independently within a collaborative, interdisciplinary team.
- Sound judgment and decision-making skills in complex situations.
- Adaptability to evolving care models, systems, and ambiguity.
Nice To Haves
- Licensed Clinical Social Worker (LCSW) or License Marriage & Family Therapist (LMFT).
- Experience with Medi-Cal managed care and Enhanced Care Management (ECM programs.
- Experience in care coordination, discharge planning, or integrated care models.
- Experience using Motivational Interviewing or similar techniques.
- Bilingual (English/Spanish) preferred.
