The Behavioral Health Care Manager (Care Manager, Telephonic Behavior Health 2) in a telephonic environment, assesses and evaluates members' needs and requirements to achieve and/or maintain an optimal wellness state. This role guides members/families toward and facilitates interaction with resources appropriate for their care and well-being. The work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action. The Behavioral Health Care Manager employs a variety of strategies, approaches, and techniques to manage a member's physical, environmental, and psycho-social health issues, identifying and resolving barriers that hinder effective care. They ensure patients are progressing towards desired outcomes by continuously monitoring patient care through assessments and evaluations and may create member care plans. This role requires understanding department, segment, and organizational strategy and operating objectives, including their linkages to related areas. Decisions are made regarding own work methods, occasionally in ambiguous situations, requiring minimal direction and receiving guidance where needed, while following established guidelines/procedures.
