Integrate behavioral health care into primary care settings to make mental health support more accessible, coordinated, and effective for patients. As a Behavioral Health Care Manager, you'll support individuals experiencing common mental health and substance use concerns by providing brief, evidence-based interventions while working closely with medical providers and psychiatric consultants as part of a collaborative care team. This role is ideal for a behavioral health professional who thrives in a fast-paced, team-based environment and is passionate about bridging the gap between physical and mental health care. You’ll manage a defined patient caseload, conducting screenings, providing short-term interventions, and tracking treatment progress over time to ensure patients are improving and staying engaged in care. Working in close coordination with physicians, psychiatrists, and other behavioral health providers, you'll help guide treatment decisions, support care transitions when needed, and connect patients with additional resources.
