About The Position

This position is being recruited and hired by Sheppard Pratt Solutions on behalf of Greater Baltimore Medical Center (GBMC). As the largest nonprofit provider of behavioral healthcare in the nation, Sheppard Pratt teams with health care systems across the country so they can provide the highest quality acute psychiatric services in their community. The Behavioral Health Care Manager position is for master’s-level, licensed clinicians. This position will join outpatient teams as a behavioral health care manager with Sheppard Pratt’s Collaborative Care Program at an assigned practice location. The Behavioral Health Care Manager will provide clinical care in collaboration with primary care providers and consulting psychiatrists. Care will consist of brief counseling, coordinating treatment, and providing proactive follow-up to treatment response utilizing a collaborative care approach.

Requirements

  • Master’s Degree in Social Work, Counseling, Mental Health, or related field.
  • Requires a current social work license (LMSW, LCSW-C) or counseling license (LGPC, LCPC) in Maryland at the time of appointment.

Responsibilities

  • Outreach to patients referred by primary care, discussing the service and scheduling.
  • Perform comprehensive psychosocial intake.
  • Develop, coordinate, and update patients' treatment plans.
  • Provide patients with brief, evidence-based psychotherapy such as cognitive behavioral therapy, motivational interviewing, behavioral activation, and problem-solving treatment.
  • Support medication management, including consultation with psychiatrist.
  • Conduct proactive follow-up to assess and document patient response to treatment.
  • Attend team meetings as required by the Program Manager.
  • Develop working relationships with primary care providers in practice to collaborate about patients and support knowledge of the current state of Collaborative Care services.
