About The Position

The Care Manager, Telephonic Behavioral Health 2, in a telephonic environment, assesses and evaluates members' needs and requirements to achieve and/or maintain an optimal wellness state by guiding members/families toward and facilitating interaction with resources appropriate for the care and well-being of members. The Care Manager, Telephonic Behavioral Health 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action. The Care Manager, Telephonic Behavioral Health 2 employs a variety of strategies, approaches, and techniques to manage a member's physical, environmental, and psycho-social health issues. Identifies and resolves barriers that hinder effective care. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations. May create member care plans. Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas. Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed. Follows established guidelines/procedures.

Requirements

  • Current valid social service license, e.g. LMSW, LCSW, LPC, LMHC
  • Master's degree in a behavioral-health field, such as social work, counseling, or related health discipline from an accredited university
  • 2+ years or more of experience working as a behavioral health professional
  • Experience with behavioral change, health promotion, and coaching
  • Proficiency with Microsoft Office Programs, e.g. Word, Excel & PowerPoint, shared systems, troubleshoot and resolve general technical issues
  • Self-provided internet service with at minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps; wireless, wired cable or DSL connection is suggested.
  • Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.

Nice To Haves

  • Experience supporting patients telephonically
  • Experience working in Community Mental Health or as part of a crisis response team or Assertive Community Treatment (ACT) team, and substance use disorder treatment
  • Experience working with both children and adults
  • Bilingual (English/Spanish)
