The Field Care Manager, Behavioral Health 2 assesses and evaluates member's needs to achieve and maintain an optimal wellness state. This role guides members/families toward appropriate resources for their care and well-being, facilitating interaction with these resources. The work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action. The Field Care Manager, Behavioral Health 2 employs a variety of strategies and techniques to manage a member's physical, environmental, and psycho-social health issues, resolving barriers that hinder care. This role ensures patient progress towards desired outcomes by managing patient care through assessments and evaluations and may create member care plans. Understanding department, segment, and organizational strategy and operating goals, including their linkages to related areas, is essential. Field work is a mandatory requirement for this position, with travel up to 25% of the time. Applicants should reside within the state of South Carolina, with preference given to those in the Upstate or Midlands area. This role is considered patient facing and is part of Humana’s Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB. This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance.
