  • Strong communication and collaboration skills, including the ability to build trust with diverse populations and navigate difficult conversations with care.
  • Ability to work across community, clinic, telehealth, and telephone settings.
  • Commitment to harm reduction, health equity, and nonjudgmental, patient-centered support.

Nice To Haves

  • California LCSW or LMFT license, or eligibility for licensure.
  • Experience with Medi-Cal or Medicaid populations, CalAIM, or ECM programs.

  • Provide direct patient support using motivational interviewing, harm reduction, and trauma-informed approaches in community, clinic, and phone settings.
  • Serve as the team resource for behavioral health questions, warm handoffs, and crisis response protocols; triage patients presenting with suicidality, acute mental health concerns, or substance use crises and connect them to appropriate care.
  • Connect patients to mental health services, peer support programs, crisis lines, and other Inland Empire community resources.
  • Coordinate whole-person care with the RN Care Manager, Care Coordinator, Community Health Worker, and Program Manager.
  • Participate in daily huddles, weekly case conferences, and care planning meetings as the behavioral health representative.
  • Support transitions and warm handoffs to county behavioral health, inpatient, and crisis services.
