About The Position

The Behavioral Health Care Coordination Specialist supports youth and families through OhioRISE care coordination. The specialist develops and implements crisis and safety plans as needed, facilitates referrals and connections to services, monitors care plan implementation, and manages 1915(c) Waiver funds while ensuring required prior authorization and Ohio Department of Medicaid approval. Services and care management must respect the youth and family’s goals, provider choices, and resources, and be family-friendly and culturally competent. The specialist conducts outreach, assessments, and meetings in person and in the community or according to member and guardian preference. Caseloads are generally one Intensive Care Coordination case per 10 members and one Moderate Care Coordination case per 20 members, with a mix possible when warranted. The specialist engages newly referred youth and families, partners with them to develop a Child and Family-Centered Care Plan (CFCCP), and helps identify the client’s family team. The role includes completing comprehensive assessments, including Child and Adolescent Needs and Strength (CANS) assessments, and facilitating Child and Family Team (CFT) meetings at least monthly or more often as needed. The specialist serves as the CFT facilitator and point person, coordinating with team members and providers to support the plan’s implementation and effectiveness. The role requires professional boundaries, communication appropriate to the patient’s level of functioning and readiness, and awareness of how personal experiences and values may affect therapeutic relationships. The specialist coordinates with providers and agencies, including physical health providers and school personnel, communicates progress and updated strategies to the team, and helps develop community resources. Care should be culturally competent and trauma-informed. The specialist documents care plans and patient responses accurately and promptly, bills for care coordination activities, maintains team attendance and CFCCP approvals in the youth’s electronic health record, and ensures the signed CFCCP is placed in the record within two weeks of the CFT meeting. The specialist also provides the CFCCP to Aetna for quality review, initiates service authorization as required within ODM timelines, and develops SMART goals with clear measures.
