About The Position

Rocky Mountain Care is looking for a compassionate, experienced RN and growth-minded clinical leader to join our growing Uintah Basin Home Health & Hospice team in Vernal, Utah. This position offers an opportunity to help grow services, develop the clinical team, and build relationships within the community. It also provides a path to increased leadership responsibility and future Clinical Director opportunities. The Vernal team provides personalized Home Health & Hospice care in patients' homes, making a meaningful difference for patients and families. Requirements

  • Active Utah RN license required.
  • Strong OASIS knowledge and clinical documentation skills.
  • Strong clinical judgment, communication, and leadership skills.
  • Ability to lead, supervise, coach, and develop clinical staff while promoting Accountability, Commitment, and Trust.
  • Strong clinical judgment with the ability to make sound, independent decisions.
  • Excellent communication and interpersonal skills, with the ability to build effective relationships with patients, families, physicians, referral partners, and team members.
  • Ability to collaborate with and gain the cooperation of others to support organizational and patient-care goals.
  • Ability to manage multiple priorities and work effectively in a fast-paced, high-stress healthcare environment.

Nice To Haves

  • Home Health, Hospice, or clinical leadership experience preferred.
  • 3–5 years of healthcare, home health, or hospice experience preferred.
  • 2+ years of supervisory or clinical leadership experience preferred.
  • Strong knowledge of Home Health & Hospice practices, clinical operations, and patient-centered care.

Responsibilities

  • Support clinical operations and patient care for Home Health & Hospice.
  • Lead, mentor, and develop clinical staff.
  • Support quality, compliance, patient outcomes, and satisfaction.
  • Build relationships with physicians, referral partners, patients, and families.
  • Help grow our Vernal Home Health & Hospice services.
  • Partner with the Clinical Director to build a strong, accountable, people-first team.
  • Participate in employee evaluations, onboarding, probationary-period monitoring, and performance development in accordance with agency policies.
