About The Position

This role involves dual responsibilities as an Assistant Coordinator and a Children's Case Manager. As an Assistant Coordinator, the individual will support the Coordinator with various administrative and operational duties related to staff service logs and progress notes. As a Children's Case Manager, the role focuses on providing community-based interventions, case management, and wraparound facilitation for children and their families. This includes assisting patients in accessing services, developing treatment plans, coordinating with various agencies, and providing direct support services.

Requirements

  • BA/BS degree or four years of equivalent education and/or experience working in the human services field.
  • Must pass KBI, DCF child & adult abuse check, motor vehicle screen, and background checks.
  • If using a personal vehicle for work, must have reliable, insured transportation that is properly registered and compliant with all applicable state and local requirements.
  • Strong communication, problem-solving, and organizational skills.
  • Ability to work collaboratively in a team environment.
  • Ability to work independently in the community.
  • Proficient with computers and word processing.
  • Regular and punctual attendance.
  • Ability to operate a vehicle as part of routine job functions.
  • Ability to use a computer and software as part of routine job functions.
  • Attendance at in-person meetings and training sessions is required.
  • Ability to lift and carry up to 25 lbs as part of routine job functions.

Nice To Haves

  • Completion of specific Medicaid State Plan Non-Waiver Services trainings (CPST, ICE, Psychosocial Rehabilitation, Targeted Case Management, Crisis Intervention).
  • Completion of HCBS Medicaid Waiver Services trainings (Attendant Care, Independent Living/Skills Building, Wrap-Around Facilitation, Parent Support and Training) as indicated by patient needs or supervisor.
  • Completion of other in-house or online trainings as determined by supervisor, including RELIAS.
  • Meeting a minimum service credit expectation.

Responsibilities

  • Assist the Coordinator with daily service logs and roll call sheets, ensuring accuracy and timely submission.
  • Review and lock staff progress notes, ensuring they meet quality standards and include all necessary information.
  • Provide training and support to staff as needed.
  • Assist with weekly shopping and preparation for group activities.
  • Provide face-to-face, community-based interventions with patients that are solution-focused and goal-directed.
  • Assist patients in identifying strategies, developing goals, and accessing resources.
  • Help patients maintain community functioning by identifying and responding to triggers and developing crisis management plans.
  • Facilitate and coordinate the team treatment planning process.
  • Monitor treatment plan due dates and schedule necessary appointments.
  • Coordinate patient treatment needs with other social service agencies, housing, employment, and medical services.
  • Assist patients in obtaining access to needed medical, educational, and other services, including benefits applications and transportation.
  • Coordinate services and supports for youth with family, significant others, and other systems of care.
  • Form wraparound teams and guide the Plan of Care process, ensuring compliance with rules.
  • Reassemble teams for reviews and build collaboration among team members.
  • Document all coordination in the youth's health record.
  • Provide or coordinate additional services such as Attendant Care, Psychosocial Rehabilitation, Independent Living/Skills Building, Parent Support and Training, Respite Care, and Crisis Intervention.
  • Document all services in the patient's electronic health record (EHR) concurrently and in accordance with policies.
  • Complete and submit month-end Patient Status Reports.
  • Operate company or personal vehicles responsibly for work-related travel.
  • Perform other duties as assigned by Supervisor or Chief Executive Officer.
