About The Position

Under the direction of the Manager and in partnership with local Network Leaders, the Assessor Coordinator Senior is an integral part of operations responsible for putting strategy into action through education, implementation, clinical support and evaluation of the home care services provided by interdisciplinary care teams. They will also have a provincial role in consultation, development, and standardization of home care programming across the Saskatchewan Health Authority. The Assessor Coordinator Senior is the clinical and program resource for all Home Care programming needs. They will collaborate with other portfolios within SHA to support Primary Health Care/Health Service Managers in ensuring front line staff have all the resources, training and clinical support they require to do their work consistently, safely, effectively, and efficiently aligning with current best practices.

Requirements

  • Two (2) years clinical experience
  • Bachelor of Social Work (BSW)
  • Valid Class 5 driver's license
  • Registered/eligible for registration with the appropriate licensing body
  • Advanced computer skills
  • Data analysis skills
  • Leadership skills such as engagement, communication, and team building
  • Ability to apply research to clinical practice
  • Ability to work with minimal supervision
  • Articulates theory and knowledge related to patient and family centered care
  • Ability to develop and implement programs / resources from needs assessments
  • Ability to establish and maintain interpersonal/consultative relationships
  • Demonstrated commitment to continuing professional development
  • Demonstrates ability to accept and implement change
  • Demonstrates effective verbal and written communication skills.
  • Knowledge of Occupational Health and Safety Standards and application of such
  • Makes judgements on the basis of critical thinking and problem solving criteria
  • Ability to be self directed and work relatively independently
  • Demonstrated ability in program development, implementation and evaluation

Nice To Haves

  • Experience using Procura and MDS-RAI assessment tools is considered an asset
  • Bachelor of Physical Therapy is acceptable
  • Undergraduate degree in Health or Social Sciences
  • Bachelor of Nursing is acceptable
  • Bachelor of Occupational Therapy is acceptable

Responsibilities
