About The Position

The Assessor Coordinator – Degree - Senior is a supervisory, team leader position in Primary Health Care, reporting directly to the Manager. The position will provide clinical supervision for Assessor Coordinators and Social Workers on the team in the functions of case management practice. The position requires the use of high order knowledge, clinical expertise and strong interpersonal skills to coach, mentor, orientate and provide education to the team members. Drawing on standards of practice for case management; professional codes of ethics and standards of practice in health care; program policy and procedures; and, relevant legislation, the incumbent will ensure that the team provides high quality client service and professional standards of practice are maintained.

Requirements

  • Registered Social Worker (RSW) or Registered Nurse (RN)
  • Valid Class 5 driver's license
  • Bachelor of Social Work (BSW) or Bachelor of Science in Nursing (BScN)
  • Time management skills
  • Demonstrates ability to show initiative and self-direction.
  • Conflict resolution skills
  • Organizational skills
  • Experience in case management
  • Minimum 2 years related clinical experience with demonstrated leadership ability
  • Professional maturity and leadership in clinical area.
  • Knowledge of how to coach for performance
  • Advanced clinical skills/knowledge/abilities for competent social work practice
  • Highly developed written and oral communication skills
  • Ability to work with minimal supervision
  • Decision making through the application of knowledge and critical thinking
  • Strong commitment to professional dev. & ongoing learning for self and others.
  • Demonstrates leadership w/ peers and stakeholder groups across continuum of care
  • Decision making through the application of clinical, legal & ethical practice
  • Ability to be self directed and work relatively independently
  • Registered/eligible with the Saskatchewan Association of Social Workers (SASW)
  • Registered/eligible with the College of Registered Nurses of Saskatchewan (CRNS)

Responsibilities

  • Provide clinical supervision for Assessor Coordinators and Social Workers on the team in the functions of case management practice.
  • Coach, mentor, orientate and provide education to team members.
  • Ensure the team provides high quality client service and professional standards of practice are maintained.
