About The Position

Vanderbilt Health is seeking an Assessment Nurse for Home Care to join their team. This role involves collaborating on home health plans, coordinating care delivery, and anticipating patient needs. The nurse will analyze patient information to determine nursing diagnoses, utilize the nursing process to evaluate progress, and implement interventions for effective outcomes. The position also focuses on promoting health and a safe environment. This is a part-time role with 32 hours per week, specifically for weekend shifts. The nursing team assists patients with daily tasks, medication management, and provides emotional support in their homes across several counties. Requirements

  • LIC-Registered Nurse - Licensure-Others
  • Relevant Work Experience
  • 1 year of experience
  • Graduate of an approved discipline specific program

Responsibilities

  • Collaborates in establishing the home health plan of care.
  • Coordinates and implements care delivery while anticipating patient care needs across the continuum.
  • Analyzes comprehensive information pertinent to the patient's care and management to determine nursing diagnosis.
  • Continually utilizes the nursing process to evaluate patient's progress towards goals and applies appropriate interventions to ensure effective outcomes.
  • Employs strategies to promote health and a safe and healthy environment.
  • Conducts primary care patient interviews.
  • Assesses the safety of the home environment.
  • Performs a comprehensive physical assessment.
  • Recognizes problems, asks questions, and contacts the patient's primary care provider.
