About The Position

The #1 Priority of this position is to coordinate the care of our patients and assist with any Medicare issues. This role involves reviewing admission documentation to ensure service quality, coordinating with field staff to ensure adherence to agency policies, and reviewing patient care plans for proper implementation. Daily communication with field staff is essential to ensure services meet patient expectations. The nurse will also review physician orders and verify the proper delivery of daily and emergent services.

Requirements

• RN License required.
• Responsible, always on time, resourceful, very accurate and detail-oriented.
• Flexible as to work schedule.
• Must be available to work on weekends, holidays and in emergency situations.
• Must be able to work under pressure and multi-task.
• Self-motivated with a strong work ethic and the ability to work independently AND as a team member.
• Demonstrate excellent verbal communications, interpersonal skills and empathetic listening.

Nice To Haves

• Home Health experience of knowledge of Clinical Terminology preferred.

Responsibilities

• Coordinate the care of our patients and assist with any Medicare issues.
• Review all admission documentation in order to ensure the quality of the services to be provided.
• Responsible for coordination with all field staff, that they follow agency’s policies and procedures.
• Responsible for reviewing the patient’s plan of care and ensuring that it gets properly implemented, including discussing it with the patients as necessary.
• Maintains daily communication with field staff in order to ensure that all services are provided as per patient’s expectations.
• Responsible for reviewing all physicians’ orders obtained by field staff.
• Verifies the proper delivery of daily services as scheduled, including unscheduled emergent visits in assigned community.
