This is a full-time position available primarily during the day. Strong Internal Medicine is seeking a motivated, team-oriented Nurse Navigator (NN), RN to join our dynamic, multidisciplinary team providing high-quality primary care to an adult population. In this critical role, the nurse will lead the coordination of clinical care within our Patient-Centered Medical Home model. Working collaboratively under the general direction of the Nurse Manager and Site Medical Director, the Nurse Navigator will play a central role in care planning, care facilitation, and proactive patient management. This includes managing access and utilization, coordinating across the healthcare continuum, and improving outcomes for our high-risk patients.

Requirements

  • RN with current New York State license required.
  • Minimum of three years of professional nursing experience required.

Nice To Haves

  • Outpatient primary care experience preferred.
  • Experience in community health, care management, transitional care and/or disease management is preferred.

Responsibilities

  • Provides individualized assistance, support and comprehensive coordination of care for patients and their families/caregivers, to eliminate barriers to timely care, facilitate flow through system, facilitate interactions with care team, increase patient and provider satisfaction, maintain point of contact with providers and serve as first point of contact for patients and families.
  • Coordinate and oversee care for patients identified as high-risk based on health risk assessments/appraisals.
  • Support patients through transitions of care by identifying gaps in care and coordinating follow-up.
  • Serve as the central point of contact for care coordination at the point of service and through targeted outreach and follow-up.
  • Facilitate access to clinical and ancillary services both within and outside the health system to support the patient’s care plan.
