About The Position

The Ambulatory Patient Navigator I supports coordinated, patient-centered care across the ambulatory Primary Care network through patient outreach, navigation, care coordination, and transitional care support activities. The role supports Advanced Primary Care Management (APCM), Transitional Care Management (TCM), preventive care initiatives, chronic disease management, referral coordination, and care gap closure workflows. It is designed to improve patient outcomes, continuity of care, quality performance, and patient experience. The Ambulatory Patient Navigator I collaborates with providers, nursing, operations, and interdisciplinary teams to support value-based care initiatives and Patient-Centered Medical Home (PCMH) objectives.

Requirements

Experience in patient outreach, navigation, care coordination, and transitional care.
Familiarity with Advanced Primary Care Management (APCM).
Familiarity with Transitional Care Management (TCM).
Experience with preventive care initiatives.
Experience with chronic disease management.
Experience with referral coordination.
Experience with care gap closure workflows.
Ability to collaborate with providers, nursing, operations, and interdisciplinary teams.
Understanding of value-based care initiatives.
Understanding of Patient-Centered Medical Home (PCMH) objectives.
