About The Position

A team-based care RN (Registered Nurse) navigator plays a pivotal role in delivering coordinated, patient-centered healthcare within a multidisciplinary team. Their responsibilities emphasize collaboration, evidence-based practice, and quality improvement. The RN Navigator is an experienced registered nurse who is accountable for the effective clinical care coordination of patients. The Navigator proactively identifies and resolves patient’s needs and clinical guidance throughout their care within Deborah Heart and Lung Center and it's practice locations. The RN navigator coordinates a plan of care across the system for Deborah patients. The RN Navigator systematically and continuously collects and assesses information related to patient health status to develop, execute, and evaluate the plan of care for patients and/or family. Serves as a single point of contact for referring physicians, patients and caregivers to provide resources and assistance with accessing clinical and supportive care services offered within Deborah Heart and Lung Center and practice locations. Work closely with physicians, nurse practitioners, medical assistants, behavioral health specialists, and other team members to provide comprehensive care.

Requirements

  • Graduate of accredited diploma, Associates degree or baccalaureate degree in nursing
  • NJ State RN license
  • BLS
  • Excellent interpersonal skills.
  • Ability to perform well in a fast-paced complex environment.
  • Computer literacy.
  • Familiarity with MS Office Applications.
  • 1-3 years Clinical experience

Nice To Haves

  • experience in cardiac, critical care and or ambulatory
  • Cardiac-specific credentials (e.g., CCRN, CHFN)
  • Ambulatory nursing preferred

Responsibilities
