Patient Navigator Ambulatory I

About The Position

The Ambulatory Patient Navigator I supports coordinated, patient-centered care across the ambulatory Primary Care network through patient outreach, navigation, care coordination, and transitional care support activities. The role supports Advanced Primary Care Management (APCM), Transitional Care Management (TCM), preventive care initiatives, chronic disease management, referral coordination, and care gap closure workflows. It is designed to improve patient outcomes, continuity of care, quality performance, and patient experience. The Ambulatory Patient Navigator I collaborates with providers, nursing, operations, and interdisciplinary teams to support value-based care initiatives and Patient-Centered Medical Home (PCMH) objectives.

Requirements

  • High School Diploma or equivalent required.
  • Minimum one (1) year of experience working in healthcare, ambulatory outpatient practices, patient access, care coordination, EMS or related field preferred.
  • Experience working within an electronic medical record preferred.
  • Strong communication and interpersonal skills.
  • Strong organizational and customer service skills.
  • Ability to manage multiple priorities in a fast-paced ambulatory environment.
  • Ability to work collaboratively within interdisciplinary teams.

Nice To Haves

  • Associate’s degree in Healthcare Administration, Public Health, or related healthcare field preferred.
  • Completion of Medical Assistant Program OR valid/active CMA Certification preferred.
  • Knowledge of ambulatory workflows, patient navigation, and value-based care initiatives preferred.
  • Bilingual abilities preferred based on practice needs.
