The Ambulatory RN Navigator for General Surgery plays a crucial role in ensuring continuity of care for patients. This position involves developing strong relationships with patients and their multidisciplinary teams to facilitate and navigate subsequent treatment and follow-up. The RN Navigator conducts comprehensive nursing assessments to identify individual patient needs, collaborates with various teams and departments to create an interdisciplinary plan of care, and provides essential support through education, addressing psychosocial needs, advocacy, and identifying barriers to care. They guide patients and families through the entire continuum of care, from diagnosis to treatment and follow-up, offering expert clinical advice and coordinating seamless, evidence-based care to achieve optimal patient outcomes. Additionally, the role involves evaluating and selecting patient education materials, providing education and outreach programs to patients, Advocate staff, and the community, and assisting leadership in program development and quality initiatives. The position requires the ability to demonstrate knowledge and skills appropriate for the age of the patients served, understanding principles of growth and development across the lifespan.
