About The Position

We are seeking a highly motivated, self-driven Registered Nurse to serve as an Oncology Nurse Navigator. This role is essential in guiding patients through the cancer care continuum, ensuring timely coordination of care, patient education, and compassionate support from diagnosis through treatment and survivorship. The ideal candidate is an independent, proactive RN with a passion for patient advocacy and care coordination. This individual must be comfortable working autonomously while collaborating closely with multidisciplinary teams to optimize patient outcomes.

Requirements

  • Minimum of two years’ clinical experience preferred.
  • Currently holds an unencolored RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
  • Obtains and subsequently maintains required department specific competencies and certifications.

Responsibilities

  • Integrates health care for patients/clients across the continuum of care.
  • Collaborates with healthcare team and other professionals to coordinate and ensure timely sequencing of care.
  • Supports patients/clients and other family members by removing barriers of care.
  • Seeks to evaluate outcomes based on an integration among established clinical, financial and utilization data.
  • Functions in structured and unstructured health care settings described as a geographic and/or situational environment that may not have established policies and procedures.
  • Utilizes independent nursing judgment when integrating health care.
  • Knowledge of and utilizes appropriate age-related patient care protocols relating to the physical and psychological needs of pediatric, adolescent, adult and geriatric patients according to department scope of service.
  • May serve as a key link between providers, clinics, insurance concerns, communication, inpatient floors and outside facilities.
  • Establishes and maintains good interpersonal relationships with those with whom has contact.
  • Able to work with growth and development needs of client populations in clinical area.
  • Coordinates, facilitates and negotiates with others.
  • Monitors, evaluates and trends patient responses utilizing structured care methodologies.
  • Work closely with ancillary and support services.
  • Ability to solve problems, identify and implement process improvements when needed.
  • Functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure.
