About The Position

Easterseals PORT Health is seeking an Alternative Family Living (AFL) Provider in Winston-Salem, North Carolina. The provider opens their private home to an individual with intellectual and developmental disabilities, and possibly mental health challenges, offering a family-home setting with 24-hour support and habilitation. Easterseals PORT Health aims to match each supported individual with a provider whose home and support are a suitable fit.

The provider offers a stable, caring home where the individual can live, grow, integrate into the community, and work toward daily living skills and independence. Depending on the individual, they may also attend a day program or receive other services. This is an independent contractor opportunity, not an employment position.
