About The Position

Easterseals PORT Health is seeking an independent contractor in North Carolina to become an Alternative Family Living (AFL) Provider. The provider opens their private home to an individual with intellectual and developmental disabilities and possibly mental health challenges, offering a stable, caring family environment and 24-hour support and habilitation. The goal is to help the individual live, grow, integrate into the community, and work toward daily living skills and independence. Depending on the individual’s needs, they may also attend a day program or receive other services in addition to residential support from the AFL Provider. This is an independent contractor opportunity, not an employment position.

Easterseals PORT Health provides disability, behavioral health, and addiction treatment services. Requirements

• Be willing to open a private home to an individual receiving support.
• CNA or DSP experience is preferred.
