About The Position

PrimeCare Medical is currently looking for an Alcohol and Substance Abuse Case Manager to work in the medical department at the Chester County Prison in West Chester, PA. This is a full time Monday- Friday position, 40 hours per week, during normal business hours. (8:00am-4:30pm).

Requirements

  • Bachelor's Degree in Social Work, Psychology, Counseling, or other related Human Services degrees with a concentration in mental health course work preferred.
  • A minimum of one (1) year clinical experience and/or experience in the Drug and Alcohol field is preferred.

Responsibilities

  • Assisting in the coordination and implementation of the behavioral health program.
  • Monitoring, coordination, and implementation of the MAT (Medication Assisted Treatment) program in accordance with NCCHC/ACA/MCCS Standards.
  • Coordination of the admission and placement of inmates/patients in alcohol and substance abuse facilities.
  • Initiating the paperwork required for the inmates/patient’s admission.
  • Documenting services provided in the electronic medical record in a complete, accurate and timely manner.
  • Monitoring inmates/patients to interventions and reporting any concerns to supervisory staff.
  • Assisting patients in developing a discharge plan to continue MAT (Medication Assisted Treatment) services in the community.
  • Assisting inmate/patient with proper medical assistance application.
  • Coordinating with community providers to assist inmates/patients in identifying resources that are available to them post-release.
  • Obtaining releases from patients to request and obtain records from patient's previous treatment providers.
  • Serving as a member of various committees within the medical unit and facility, as necessary.
  • Participating in all in-service education programs according to the annual in-service calendar.
  • Maintaining confidentiality at ALL times.
  • Any/all other duties and responsibilities as assigned or delegated.
