About The Position

The substance use case manager will provide comprehensive case management services to individuals with substance use disorders. This role is specifically for the Contingency Management Program which works with individuals that struggle with stimulant use disorder. This role will focus on assessing client needs, developing individualized service plans, coordinating treatment, and support services. This will help promote long-term recovery. The case manager will work collaboratively with the senior counselor, client, families, treatment providers, and community resources to ensure continuity of care and support harm reduction, recovery oriented goals. This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to teamwork and cooperation.

Requirements

Bachelor's degree in related field required.
Previous case management experience required.
Valid Driver's License required.
Ability to pass a background check through fingerprint process and drug screen as required.

Nice To Haves

CADC preferred.
Annual, American Heart Association BLS and Heart Saver First Aid required within 90 days of hire or transfer.
CPI required within 90 days of hire or transfer.

Responsibilities

Conduct initial screenings and ongoing needs assessments.
Develop, implement, and monitor individualized treatment and recovery service plans.
Provides crisis intervention, individual counseling, and strength-based support.
Facilitate referrals for mental health, housing, employment, court, and other supportive services.
Monitor client progress, provide follow-up, and adjust service plans as needed.
Coordinate or provide transportation for clients when necessary.
Maintain accurate timely confidential documentation in compliance with HIPPAA, state regulations and AtlantiCare addiction services policies.
Ensure compliance with grant requirements, program standards, and funding.
