About The Position

This is a paraprofessional position within the Social Services Department, primarily supporting the Intake/Assessment Unit, with responsibilities extending to the Care Management Unit and Protective Services Unit. The role involves assisting staff by managing telephone communications, conducting field visits, and providing administrative support. The geographic area covered includes Luzerne and Wyoming counties. The employee reports to an Aging Care Management Supervisor 1.

Requirements

  • One year as an Aging Case Aide 1; OR Two years of public contact work, including one year in a human services program; OR Any equivalent combination of experience and training.
  • Be a resident of Pennsylvania.
  • Meet the minimum experience and training required for the job.
  • Meritorious Service: Absence of any discipline above the level of written reprimand during the 12 months preceding the closing date of the posting.
  • Last due overall regular or probationary performance evaluation was higher than unsatisfactory.
  • Seniority: A minimum of twelve months in the next lower class(es) by the closing date of this posting.
  • Knowledge of the physical, cultural and social aspects of the assigned community.
  • Ability to obtain, evaluate and analyze information from interviews, reports, records and other sources.
  • Ability to provide information to clients and other personnel.
  • Ability to communicate effectively, both orally and in writing.
  • Ability to establish and maintain positive working relationships.
  • Ability to learn about Area Agency on Aging Programs and the services of other community agencies.
  • Ability to effectively communicate verbally with older persons.
  • Ability to read, write, and perform simple arithmetic computations.
  • Ability to follow simple oral and written instructions.
  • Ability to express ideas clearly and persuasively to elderly clients.

Responsibilities

  • Maintain on-going proficiency in the use of the statewide Department of Aging data bases (Well-Sky and PIA).
  • Contact consumers by telephone to assist in screening referrals for Assessment and diverting to the appropriate department.
  • Record responses to follow-up telephone calls in a standardized format in the journal section of the AAA Data Base.
  • Contact the assigned Care Manager with any issues or concerns.
  • Field all in-coming telephone calls from consumers regarding Aging Programs (Needs Assessments, Functional Eligibility Determinations, home modifications, OPTIONS, etc.).
  • Complete request for assistance forms for consumers who walk into the AAA.
  • Conduct home visits to assist consumers in gathering financial documentation or other application information.
  • Assist with Functional Eligibility Determinations by performing Desk Reviews and compilation of assignment lists.
  • Telephone consumers as assigned for special projects (e.g., emergency situations, waiting list status, follow-up calls for unmet needs, financial documentation requests).
  • Pick up or deliver consumer necessities as requested by Care Managers and Supervisors (e.g., emergency medical supplies, emergency food items, paperwork).
  • Assist the intake unit as warranted due to call volume or coverage needs.
  • Keep track of all activities assigned each month and consumers seen each month for statistical purposes.
  • Perform administrative tasks such as Xeroxing, Faxing, Scanning, Filing, Mailings, Shredding, and Purging.
  • Attend meetings, in-services, seminars, and webinars as required.
  • Perform other duties as assigned by the Supervisor.
