About The Position

The Aging Case Aide 2 – Protective Services position, within the Protective Services unit of the Area Agency on Aging (AAA) department, provides support to Protective Services Investigators and Protective Services Supervisors. This role involves assisting with financial information gathering, coordinating services, maintaining resource information, and documenting case activity. The position requires travel to interact with consumers and community representatives, and may involve working in various environments.

Requirements

One (1) year as a Case Aide 1 (Local government); OR Two (2) years of public contact work, including one (1) year in a human services program; OR Any equivalent combination of experience and training.
Must possess and maintain a valid and current motor vehicle operator’s license.
Must have an acceptable driving record.
Personal vehicle required for County business.
Knowledge of the physical, cultural, and social aspects of the assigned community.
Ability to obtain, evaluate, and analyze information from interviews, reports, records, and other type of sources of information, and to provide information to clients and other types of personnel.
Ability to establish and maintain positive working relationships.
Ability to learn about Area Agency on Aging Programs and the services of other community agencies.
Ability to communicate verbally with older persons.
Ability to perform simple arithmetic computations.
Ability to follow simple verbal and written instructions.
Ability to express ideas clearly and persuasively to elderly clients.
Ability to clearly express ideas verbally, and in writing.
Ability to maintain confidentiality at all times.
Ability to communicate fluently in English, including telephone contacts, face-to-face contacts, and communications in large and small group settings.
Ability to read the English writing in cursive and printed forms.
Ability to analyze and interpret information that is received in written and verbal form.
Travels to and interacts with consumers, members of the community, and representatives of other social agencies in their homes, offices, and other community locations.
Attends training as required either locally or out of town.

Responsibilities

Attends supervisory conferences/unit meetings as scheduled.
Attends training, locally or out-of-town, as approved by the Supervisor.
Cooperates with co-workers and staff of other divisions of the Department of Human Services and community agencies in the provision of service to shared consumers when appropriate.
Serves as a contact for banks.
Develops relationships with key personnel at financial institutions.
Assists with gathering financial information on financial exploitation cases and obtain financial statements when indicated.
Accompanies consumers to bank if necessary to receive bank records.
Obtains state reporting forms and incident reports from facilities (nursing homes, assisted livings, personal care homes, and adult day care centers).
Completes home visits as directed to assist with various errands such as picking up paperwork, dropping off food, etc.
Keeps resource information current.
Provides assistance to Protective Services staff with tasks such as taking RONs for Protective Services, processing and tracking assignment of RONs, and completing required notifications.
Enters RONs and/or progress notes for PS Care Managers and/or PS Supervisors when needed.
Provides telephone contact support to implement and/or ensure follow through with designated areas of consumers’ care plans as directed by PS Unit Director or PS Supervisors.
Follows up with physicians to obtain reports and/or schedule competency evaluations.
Orders necessary formal/contract services and process service notifications in Social Assistance Management System (SAMS).
Reestablishes support networks for isolated consumers.
Assists with the relocation process, i.e., complete housing or NH applications, and follow-up with facilities, etc., as well as arrange emergency shelter in interim.
Assists PS Aging Care Managers with gathering other necessary information.
Faxes and e-mails information as directed.
Checks fax folders and distributes faxes.
Documents all case activity in SAMS PS Care Plan Journal.
Completes necessary steps to close the consumer to PS as needed.
Completes filing as directed.
Assists with running reports in SAMS, such as Reports of Need (RONs) and Protective Services Investigator (PSI) missing data reports.
Deletes and shreds files in accordance with PS regulations.
