About The Position

This position is responsible to provide information and assistance, program eligibility determination and comprehensive case management services to older adults and families eligible for federal, state and county programs related with the Older Americans Act and within the scope of the Aging & Disability Resource Center. This position serves as the division’s representative for caregiver support programs.

Requirements

  • Master’s degree (bachelor's degree with experience accepted) in social work or related field and at least one year of relevant prior experience.
  • Proven experience in ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals as well as ability to compute rate, ratio and percent and to draw and interpret bar graphs.
  • Knowledge of Trauma Informed Care and Health Equity Principles is essential.
  • Valid driver’s license
  • Proof of minimum liability auto insurance coverage.
  • Must be able to provide a reliable personal vehicle to be used for business related travel as needed.

Responsibilities

  • Conduct comprehensive initial assessments at the time of consumer admission into Older American Act funded programs.
  • Perform ongoing assessment updates to accurately reflect the consumer's status, as required by program mandates.
  • Provide case management services for assigned caseload, including face-to-face contact, in home or community.
  • Evaluate and assess individuals/families with psychosocial/physical challenges, develop service plans, carry out plan, coordinate and monitor services/treatment, review and term services.
  • Collaborate with contracted providers and closely monitor purchased services that are provided to consumers.
  • Based on assessment findings, make a determination regarding ongoing need and effectiveness of provided services.
  • Consult, coach and collaborate with team member/families/individual/providers/HHS staff.
  • Collaborate with other programs/individuals across the division to ensure coordination of care, with priority to the Aging & Disability Resource Center and Adult Protective Services units.
  • Provide oversight and facilitation of Wisconsin’s Family Caregiver Support Programs – NFCSP and AFCSP.
  • Participate in the State Caregiver Support Network and educate caregivers about available resources.
  • Facilitate consumer access to service.
  • Coordinate and monitor services in a timely manner.
  • Incorporate current best practice models into daily service provisions; work with individuals/families.
  • Participate in trainings to update knowledge and skills to maintain standards and ensure certification/personal license.
  • Ensure compliance with program standards and maintain necessary documentation as required by Walworth County Department of Health & Human Services, Department of Health Services and state/federal statutes.
  • Participate in Long Term Care division functions related to the general operation including trainings, supervision, progress meetings for consumers, division/program staff meetings, supervision, resource development, marketing and outreach to the general public.
  • Serve on related work groups, projects, committees and/or community projects, as assigned.
  • Demonstrate a commitment to county safety and risk management efforts.
