The Lebanon County Area Agency on Aging is seeking a motivated Aging Case Aide 2 who wants to play an important role in helping older adults access vital services. Requirements

• One year as an Aging Case Aide 1; or Two years of public contact work, including one year in a human services program; or Any equivalent combination of experience and training.
• Must meet the PA residency requirement.
• Must be able to perform essential job functions.
• A conditional offer of employment will require submission of criminal history reports.

Nice To Haves

• Do not possess an active insurance license.

Responsibilities

• Handle information and referral calls and direct consumers to the proper staff
• Maintain consumer files, oversee expungements, and track weekly obituary information for case updates
• Assist care managers by completing scheduled telephone calls for ongoing case management
• Support PA Medi activities including trainings, volunteer recruitment, outreach tasks, and report submission
• Help consumers with filing insurance appeals and completing required forms and packets
• Conduct visits, monitor providers, and support moves for individuals in the Domiciliary Care program
