Requirements

  • Three years as an Aging Case Aide 2
  • A bachelor's degree which includes or is supplemented by twelve (12) credits in social sciences, behavioral sciences, human services, or a closely related field
  • An equivalent combination of experience and training
  • Applicants will be considered to have met the educational requirements once they are within three months of graduating with a qualifying degree
  • You must meet the PA residency requirement.
  • You must be able to perform essential job functions.
  • A conditional offer of employment will require a medical exam and a drug screening.
  • This position falls under the provisions of the Child Protective Services Law. Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.

Responsibilities

  • Complete timely annual and bi-annual recertifications and assessments, maintain lists, send advance notices to providers, and help consumers prepare and submit eligibility forms
  • Develop care plans, link consumers with appropriate service agencies and care placements, enter service details into required systems, and conduct routine visits and monitoring to ensure compliance with regulations and care plan effectiveness
  • Explain regulations, policies, and programs as well as provide counseling on available services to consumers, their families, and community members
  • Participate in individual coaching, group instruction, case reviews, professional reading, training sessions, and ongoing supervision to support your continued growth and success in the role
  • As experience is gained and proficiency is demonstrated, employees are assigned progressively more complex responsibilities, provided with increased independence in judgment and decision-making, and earn eligibility for promotions over time
