Aging Care Manager 2 (Local Government) - Wayne County Area Agency on Aging
About The Position
As an Aging Care Manager 2 with the Wayne County Agency on Aging, you help people access support that improves their quality of life and supports the independence and dignity of older adults and others in need. You assess consumer needs, arrange services, and explain available programs, guiding people through agency processes with strong organization, communication, and problem-solving skills. The position is full-time, with work hours of 8:00 AM to 4:30 PM, Monday through Friday, including a 30-minute lunch. On-call hours may be required. Overnight or out-of-county travel for meetings or training may occur. Telework is not available.
Requirements
- Meet one of the stated minimum experience and training pathways: six months as an Aging Care Manager 1 or County Caseworker 1; successful completion of the County Social Casework Intern program; or six months of professional human services or nursing experience and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or an equivalent combination of experience and training.
- Meet the Pennsylvania residency requirement.
- Be able to perform the essential job functions.
- Pass a background investigation.
- Submit to a drug screening upon conditional offer of employment.
- Submit satisfactory criminal history clearances required under the Child Protective Services Law, including Pennsylvania State Police, Pennsylvania Child Abuse history, and FBI Fingerprint clearances.
Responsibilities
- Assess consumer needs, maintain records, and complete reassessments to ensure appropriate services.
- Arrange and coordinate services with community resources, hospitals, and agency programs.
- Screen consumers for program eligibility and explain cost sharing, donation procedures, and appeals.
- Collect required documents and obtain written consent to access or share client information.
- Explain service options, associated costs, and available alternatives to consumers.
- Perform intake and referral responsibilities, ensuring accurate and timely documentation.
