Requirements
- Six months as an Aging Care Manager 1 or a County Caseworker 1; or Successful completion of the County Social Casework Intern program; or Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or An equivalent combination of experience and training.
- Meet the PA residency requirement.
- Be able to perform essential job functions.
- Pass a background investigation.
- A conditional offer of employment will require a drug screening.
- This position falls under the provisions of the Child Protective Services Law. Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
Responsibilities
- Gather information from consumers and provide referrals and explanations of available services
- Arrange and monitor services for consumers with simple to complex needs
- Conduct initial and annual assessments in homes, facilities, hospitals, or by phone
- Document case activity, complete reports, and maintain accurate files
- Work with families, providers, and community agencies to support consumer needs
- Travel within the county to meet consumers in varied locations and conditions
