About The Position

Located in Hawley, Wayne County, PA—this Aging Case Aide 2 position is a rewarding opportunity for a patient, compassionate, and organized individual who enjoys working with senior citizens and supporting their independence and well-being. With our Area Agency on Aging, you will join the collaborative team at our Adult Daily Living Center (ADLC) and Senior Center in Hawley and assist at other senior centers or satellite as needed. Duties include assisting senior day center participants with personal care and daily needs while promoting safety, dignity, and independence. Wayne County offers an excellent benefits package included, along with meaningful work serving older adults in the community. This position participates in the direct community and personal care support provided by the Hawley ADLC and Senior Center in addition to supporting our agency's work and community outreach at other locations as needed. On the job training and in-service training sessions and seminars are provided.

Requirements

  • One year as an Aging Case Aide 1; or Two years of public contact work, including one year in a human services program; or An equivalent combination of experience and training.
  • Meet the PA residency requirement.
  • Able to perform essential job functions.
  • Pass a background investigation.
  • A conditional offer of employment will require a medical exam and a drug screening.
  • Must submit and have approved satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.

Responsibilities

  • Provide information, assistance, and benefits counseling to older adults, including Medicare/PA MEDI, PACE, LIHEAP, and Tax/Rent Rebate programs
  • Assist with application completion, referrals to community resources, and coordination with agency care management
  • Perform routine non-professional care services for participants with significant cognitive and/or physical disabilities; support daily nutrition services including meal distribution and sanitation procedures
  • Coordinate senior center programs, activities, and special events; recruit and supervise volunteers; assist with member registration and assessments; and collect and document donations
  • Conduct outreach throughout Wayne County, including home visits to homebound clients; assist with newsletters, informational displays, transportation arrangements, and Medicare Part D education events
  • Maintain records and perform data entry in agency and statewide databases, including PA MEDI and Information & Referral documentation
