Aging Care Manager 2 (Local Government)
About The Position
Westmoreland County Area Agency on Aging is seeking a full-time Aging Care Manager II to help older and disabled persons live as independently as their circumstances allow. The position focuses on protecting older adults and supporting their ability to remain safe in the community. The work includes investigating possible abuse, neglect, exploitation, or abandonment; assessing safety concerns; coordinating services and interventions; responding to urgent situations; participating in legal proceedings; and monitoring case plans and supports. The work schedule is 8:30 AM to 4:00 PM, Monday through Friday, with a 30-minute lunch. The employee may be required to participate in an off-hours standby rotation. Telework is not available.
Requirements
- Meet one of the minimum experience and training pathways: six months as an Aging Care Manager 1 or County Caseworker 1; successful completion of the County Social Casework Intern program; or six months of professional human services or nursing experience and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or an equivalent combination of experience and training.

Responsibilities
- Investigate reports involving possible abuse, neglect, exploitation, or abandonment of older adults.
- Interview consumers, families, and professionals and collect information to assess safety concerns.
- Coordinate with medical, legal, and social service professionals to reduce risks and support necessary interventions.
- Respond during or after normal work hours to urgent protective situations.
- Prepare documentation and testify in legal proceedings related to protective service cases.
- Follow up on plans and supports to help consumers maintain safety and reduce risks.
