Requirements
- Three years as an Aging Case Aide 2; or A bachelor's degree which includes or is supplemented by twelve (12) credits in social sciences, behavioral sciences, human services, or a closely related field; or An equivalent combination of experience and training.
- Applicants will be considered to have met the educational requirements once they are within 3 months of graduating with a qualifying degree/credit.
- You must meet the PA residency requirement.
- You must be able to perform essential job functions.
- Conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
- Conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.

Responsibilities
- Assess consumer needs and create care plans based on social contact, daily activities, cognitive status, physical health, and available resources
- Collect details from consumers and their support networks to identify concerns and determine needed services
- Connect individuals with community programs and resources that address their unique situations
- Help consumers manage income to support their daily living needs
- Use the State reporting system to enter accurate case information and updates
