About The Position

TFI Family Services is hiring a full-time Aftercare Case Manager to support children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aging out of care. The role focuses on risk management and helping families maintain stability during the aftercare period. It includes crisis intervention, coordination with public and private agencies, maintaining documentation and reports, connecting families with community and educational services, and meeting with families in person to support reunification and address concerns. The average hourly rate is $19.23, including wage, incentives, bonuses, overtime, shift differential, and other listed components.

Requirements

• Bachelor’s degree in human services.
• Valid driver’s license.
• Willingness to learn; training is provided.
• Compassion and commitment to strengthening families.
• Eagerness to learn and grow skills to better support families and children.
• Complete a Predictive Index assessment; it takes 5–7 minutes.

Responsibilities

• Focus on risk management and help families maintain stability during the aftercare period.
• Provide crisis intervention and work with public and private agencies offering supportive services.
• Maintain accurate documentation and reports.
• Coordinate community and educational services for families.
• Meet with families in person to support reunification and resolve concerns.
• Help families connect with appropriate community resources to maintain stability.
• Advocate for the families and children served.
