About The Position

TFI Family Services is hiring a full-time Aftercare Case Manager to support children and families who have achieved permanency through reintegration, adoption, custodianship or guardianship, or aging out of care. The role focuses on risk management and helping families maintain stability during the aftercare period. It includes providing crisis intervention, coordinating supportive community and educational services, maintaining documentation, and meeting with families in person to support reunification and address concerns.

The ideal candidate is compassionate, committed to strengthening families, able to help families connect with community resources, and eager to develop skills to better support children and families. Training is provided. The average hourly rate is $19.23, including wages, incentives, bonuses, overtime, and shift differential.

Requirements

• Bachelor’s degree in human services.
• Valid driver’s license.
• Willingness to learn; training is provided.
• Complete a Predictive Index assessment, which takes approximately 5–7 minutes.

Responsibilities

• Focus on risk management and help families maintain stability during the aftercare period.
• Provide crisis intervention and work with public and private agencies that offer supportive services.
• Maintain accurate documentation and reports.
• Coordinate community and educational services for families.
• Meet with families in person to support reunification and resolve concerns.
