About The Position

TFI Family Services is hiring a full-time Aftercare Case Manager to support children and families who have achieved permanency through reintegration, adoption, custodianship or guardianship, or aging out of care. The role focuses on risk management and helping families maintain stability during the aftercare period. It includes crisis intervention, coordination with public and private agencies, accurate documentation, and arranging community and educational services. The case manager meets with families in person to support reunification and address concerns. The ideal candidate is compassionate, committed to strengthening families, able to connect families with community resources, and eager to learn and grow. TFI provides training. The position offers an average hourly rate of $19.23, including wages, incentives, bonuses, overtime, and shift differential. TFI Family Services is an Equal Opportunity Employer.

Requirements

Bachelor’s degree in human services.
Valid driver’s license.
Willingness to learn and participate in provided training.
Compassion and commitment to strengthening families.
Ability to help families connect with appropriate community resources.

Responsibilities
