About The Position

TFI Family Services is hiring a full-time Aftercare Case Manager to support children and families who have achieved permanency through reintegration, adoption, custodianship or guardianship, or aging out of care. The role focuses on risk management and helping families maintain stability during the aftercare period. The case manager provides crisis intervention, works with public and private agencies that offer supportive services, maintains documentation and reports, coordinates community and educational services, and meets with families in person to support reunification and resolve concerns.

The position seeks a compassionate advocate committed to strengthening families, connecting them with community resources, and continuing to learn and grow. Training is provided. The average hourly rate is $19.23, including wage, incentives, bonuses, overtime, and shift differential.

Requirements

• Bachelor’s degree in human services.
• Valid driver’s license.
• Willingness to learn; training is provided.

• Eagerness to learn and grow skills to better support families and children.

Responsibilities

• Focus on risk management and help families maintain stability during the aftercare period.
