Advanced Practice Provider - Heart Failure, CHVH
About The Position
Community Physician Network is seeking an experienced Nurse Practitioner or Physician Assistant to work alongside its General Cardiology team at Community Heart and Vascular Hospital on the north campus in Indianapolis, Indiana. The role is part of Community Health Network, a not-for-profit, provider-led, multi-specialty healthcare system with more than 1,400 providers. The position uses a multidisciplinary approach alongside specialty physicians and advanced practice providers. It is full-time, day schedule, with weekend and call rotations; the frequency of those rotations is determined by the practice. Experience in Advanced Heart Failure is strongly preferred. New graduate Nurse Practitioners may be considered if they have at least 3 to 5 years of RN experience in inpatient cardiac or ICU settings. Applicants must have an educational background consistent with Indiana licensure and certification requirements.
