Family Medicine - Advanced Practice Provider
About The Position
Carle Health is seeking an Advanced Practice Provider to join its Family Medicine team at Carle Health Family Medicine Pear Tree Clinic in Peoria, Illinois. The position joins an established practice and works directly with the site's lead Family Medicine physician to provide collaborative care and support for an established patient panel. Clinic hours are Monday through Friday, 8:00 AM to 4:30 PM. The clinic is part of an integrated system using Epic EMR across its clinics and hospitals to support care coordination. Carle Health is a not-for-profit organization, and this role is eligible for Public Service Loan Forgiveness (PSLF). Peoria is located on the Illinois River and offers dining and entertainment venues, nearby residential communities, and highly ranked public school districts. Carle Health has more than 17,000 team members and providers serving central and southeastern Illinois, with seven hospitals across eight campuses and a multispecialty provider group. The organization also supports healthcare education through Carle Illinois College of Medicine and Methodist College.
Requirements
- Master's degree in Nursing or Physician Assistant Studies.
- Current Illinois Registered Nurse license and Advanced Practice Registered Nurse licensure, or Physician Assistant licensure.
Responsibilities
- Provide collaborative Family Medicine care in support of an established patient panel.
