About The Position

Intermountain Health is seeking an experienced Nurse Practitioner or Physician Assistant for our Heart Failure Clinic. The selected candidate will partner with the a certified Heart Failure Cardiologist to continue to build a pre-eminent Heart Failure Program in the state of Colorado. You would practice at our Wheat Ridge Cardiology clinic and Lutheran Hospital. Additional locations may be may be added with program growth. Hours are 4/10’s Mon-Fri, no call and no weekends. Your day off will vary. You would work with stage 3 and 4 heart failure patients to ensure medication compliance, monitoring dietary and lifestyle changes, regular follow ups and much more. This position would be part of the continuity of patient care from hospital to clinic and back to the primary care physician or cardiology service line. You would work a combination of outpatient clinic consults and follow ups along with inpatient rounding and consults.

Requirements

Graduate from an accredited Physician assistant or Nurse Practitioner program
Licensed as an NP or PA in the state of CO or able to obtain
DEA and full prescriptive authority in CO or able to obtain
BLS and ACLS required
Minimum of 2 years of experience as an NP or PA seeing Heart Failure patients

Responsibilities

Partner with a certified Heart Failure Cardiologist to build a pre-eminent Heart Failure Program.
Work with stage 3 and 4 heart failure patients to ensure medication compliance.
Monitor dietary and lifestyle changes for heart failure patients.
Conduct regular follow-ups with heart failure patients.
Ensure continuity of patient care from hospital to clinic and back to the primary care physician or cardiology service line.
Work a combination of outpatient clinic consults and follow-ups.
Perform inpatient rounding and consults.
