Advanced Practice Provider (APRN/PA) - Cardiology/Heart Failure - Elmhurst Hospital
About The Position
Endeavor Health is hiring an Advanced Practice Provider (APN or PA) for its Center for Cardiac Health–Heart Failure team at Elmhurst Hospital in Elmhurst, Illinois. This is a full-time outpatient role averaging 40 hours per week, with hours Monday through Friday between 7:00 a.m. and 5:30 p.m. The provider delivers patient-centered care to high-acuity patients in an autonomous role under the direction of a collaborating physician. No travel is required. The position includes evaluating and managing patients with acute and chronic conditions, ordering and interpreting diagnostic tests, and developing diagnoses and evidence-based treatment plans. The provider also educates patients and families, coordinates care with physicians and interdisciplinary teams, supports post-discharge and transitional care, contributes to quality and safety initiatives, and helps advance organizational and service-line goals while following Endeavor Health policies and standards.
Requirements
- Active Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in Illinois.
- Federal DEA and Illinois Controlled Substance License.
- Completion of a master’s-level program in a specialty area.
- BLS/CPR certification.
- Prior APN/PA cardiology experience.
- APP experience in cardiology.

Responsibilities
- Obtain comprehensive patient histories and perform physical examinations.
- Order and interpret diagnostic tests.
- Develop differential diagnoses and prescribe evidence-based pharmacological and nonpharmacological treatments.
- Manage acute and chronic conditions.
- Provide ongoing education to patients and their families.
- Coordinate care with physicians and interdisciplinary teams to optimize outcomes and support post-discharge follow-up and transitional care.
- Support quality-improvement and patient-safety initiatives.
- Contribute to the development and implementation of organizational and service-line goals.
