About The Position

The Advanced Practice Provider (APP) reports to the Manager for CVI APPs and the ambulatory medical director. The APP works as an integral member of the advanced heart failure team working with a group of heart failure cardiologists, fellows, nurses, pharmacists, and medical assistants. This is a unique hybrid role incorporating inpatient and outpatient practice, caring for complex advanced heart failure patients. The APP participates in a busy inpatient heart failure consult service as a shared visit with a group of heart failure cardiologists. They assist in the management of advanced heart failure patients including patients in cardiogenic shock, patients requiring mechanical circulatory support, cardiac transplant patients, patients with Left Ventricular Assist Devices (LVAD), and refractory heart failure patients. The APP manages a panel of advanced heart failure patients independently in the ambulatory setting at the Advanced Cardiac Care Center at Rhode Island Hospital.

Requirements

  • Graduated from an accredited physician assistant program with a master’s degree and has current board certification from the NCCPA or graduated from an accredited nurse practitioner master’s degree program and has current board certification from ANCC or AANP.
  • Current Rhode Island state licensure to practice as an advanced practice registered nurse or physician assistant.
  • Current prescriptive privileges and DEA licenses.
  • Current BLS and ACLS certifications.

Nice To Haves

  • 1-2 years of cardiology experience as an advanced practice provider (NP or PA).

Responsibilities

  • Performs independent outpatient office visits and manages a panel of patients including heart failure hospital discharges, acute and chronic heart failure patients, Left Ventricular Assist Device (LVAD) patients, heart transplant recipients, patients listed for heart transplant, and patients on chronic inotropic support.
  • Understands heart failure disease processes, interprets laboratory results and diagnostic studies (including but not limited to Echocardiograms, Cardiac Catheterizations, cardiac MRIs, and stress testing).
  • Assesses and applies evidence-based medicine in conjunction with ACC/AHA heart failure practice guidelines.
  • Provides patient and family education regarding heart failure disease process and medications.
  • Participates in the advanced heart failure consultation service at Rhode Island Hospital.
  • Carries the heart failure service pager and responds to inpatient heart failure consults.
  • Makes assessments and develops plans of care for heart failure patients as part of a shared visit with a heart failure cardiologist.
  • Coordinates and collaborates with other members of the heart failure team as well as the hospital care team.
  • Participates in weekly heart failure team meetings.
  • Completes yearly mandatory continuing education requirements including BLS and ACLS.
  • Participates in the education, clinical research, and mentoring of healthcare providers.
  • Performs other duties as assigned.
