About The Position

Performs full performance level eligibility determination for public assistance programs. Eligibility programs include Old Age Pension, Aid to the Needy Disabled, Colorado Works, Medicaid, Supplemental Nutrition Assistance Program (SNAP), Low Income Assistance Program (LEAP) and/or Child Care and other programs as needed. Provides ongoing case management to Long-Term Care (LTC) and Adult Program clients.

Requirements

  • High school diploma or equivalent.
  • Two years of experience working in eligibility programs.
  • Valid driver’s license.
  • Ability to utilize the technology required of the position and demonstrates a willingness to learn new technologies.
  • Proficient in communication, and able to communicate using the English language, including written, verbal, or other communication channels as required by the position.
  • Maintain a high level of confidentiality and professionalism and follow through with clients, peers, providers, and supervisors.
  • Ability to be mobile during facility and house visits which may include walking on uneven surfaces.
  • Stamina to maintain focus, productivity, and attention to detail while working at a desk for standard office hours.
  • Pre-employment background check, which will include a Criminal Background Check, Employment Verification, and Motor Vehicle Record.

Responsibilities

  • Determining financial eligibility for public assistance programs under minimal supervision.
  • Interviewing clients and reviewing applications for public assistance programs eligibility.
  • Handling more complex cases independently.
  • Reviewing and verifying all information required by state and Federal regulations.
  • Completing re-determination on each case every 6 to 12 months, depending on the program, verifying and re-reviewing all information.
  • Performing customer service-focused client interactions, including home visits, client interviews, and appropriate business communications (phone calls, emailing, mailing of forms).
  • Providing exceptional customer service and ensuring timely completion of client-based tasks, community questions, or provider requests.
  • Inputting new cases into the state CBMS system and updating all information changes to current cases.
  • Establishing and creating claims to recover monies paid to clients that were incorrectly issued due to reporting or recording errors.
  • Explaining rules and regulations of all programs in detail to clients.
  • Organizing and filing cases within the CBMS system.
  • Responding immediately to phone calls from clients, providers, and the community.
  • Assisting clients and supporting with completion and filling of forms to Action Review Group (ARG), Social Security Administration (SSA), and Long-Term Care (LTC).
  • Referring clients to relevant community resources for ongoing support.
  • Preparing and mailing various forms and letters to clients.
  • Regularly attending ongoing training sessions to stay updated on State Regulations or system changes affecting all programs.
  • Working closely and maintaining regular communication with clients, authorized representatives, Single Entry Point Case Managers, nursing homes & assisted living facilities.
  • Assessing Long-Term Care participants to determine their abilities to meet ongoing case requirements.
  • Reviewing and forwarding clients' income trust documentation, along with all relevant legal assets/resources supporting documents, to the HCPF Income Trust Unit and ensuring timely follow-up.
  • Preparing 5615 forms for nursing homes and info-shares for Single Entry Point Case Managers as official billing documents to determine clients’ payment to the facility.
  • Reviewing all work products thoroughly to maintain a high level of quality.
  • Performing other duties as assigned.
