Position Summary: We are seeking a dedicated and compassionate Case Manager to join our behavioral health team, providing comprehensive case management services to individuals with Serious Mental Illnesses (SMI). The Case Manager will support consumers in achieving greater independence and improved quality of life by coordinating care, advocating for services, and ensuring ongoing access to necessary resources.

Requirements

  • Bachelor's degree in a related field (e.g., social work, psychology, sociology, counseling).
  • Experience in case management or a related field, preferably with individuals diagnosed with Serious Mental Illnesses (SMI).
  • Knowledge of community resources and social support systems.
  • Strong assessment, planning, and advocacy skills.
  • Excellent communication and interpersonal skills.
  • Ability to maintain accurate and timely documentation.
  • Valid driver's license and reliable transportation.
  • Ability to conduct home visits and community outreach.

Nice To Haves

  • Master's degree in a related field.
  • Licensure as a social worker, counselor, or related profession.
  • Bilingual abilities.

Responsibilities

  • Provide direct case management services to individuals diagnosed with SMI, including assessment, service planning, referral, linkage, monitoring, and advocacy.
  • Develop and implement individualized service plans in collaboration with consumers, families, and interdisciplinary teams.
  • Assist clients in accessing mental health, medical, housing, employment, educational, and social support services.
  • Conduct home visits, community outreach, and transport clients to appointments and community resources as needed.
  • Monitor consumer progress and update service plans accordingly.
  • Maintain accurate and timely documentation in compliance with agency and regulatory standards.
  • Participate in multidisciplinary team meetings, supervision, and case reviews.
  • Ensure coordination of care across service systems to support holistic client wellness.
