Adult Case Manager/Residential Support Worker (MHRTC)
About The Position
This is a unique opportunity to join a Residential Team in providing a combination of Case Management to Community Rehabilitation Service clients as well as Residential Support to other clients in a Group Home at the same location. This Case Management/RSW position works with other members of the team who provide services to adults with severe and persistent mental illness living in a Shalom House program. The Case Management/RSW position will work to help coordinate and collaborate with other providers to ensure that our client's housing, medical, behavioral health, and community needs are being addressed. This will be done through participation in an interdisciplinary team model to develop Treatment Plans with clients and provide a variety of services and support to assist them in meeting their identified goals. The Case Management/RSW position also assists in the Group Home working on client care and teaching coping skills. There are plenty of free training opportunities and a supportive, collaborative team. MHRT1 and CRMA training will be provided. Low Case Manager Caseloads 7-10, and position also provides support and skill building to PNMI clients in the same building as a Residential Support Worker. All clients in one building in Portland. Mileage reimbursement .58 cents/mile.
Requirements
- Must have either MHRT-C certification or a MHRTC Provisional.
- Ability to pass a criminal background check.
- Must have a clean driving record.

- MHRT1 and CRMA training provided.
- Full MHRT-C certification training - SHALOM WILL HELP PAY FOR TRAININGS FOR CANDIDATES WITH A PROVISIONAL.
