About The Position

Crossroads Turning Points (CTP) is seeking a dynamic individual to serve as the Complex Case Manager for Level 1 or 2 for the Lead Program. The PREVAIL (Pre-Arrest Evaluation, Intervention, and Linkage) program helps people avoid arrest, court, and jail when their negative behavior is driven by substance use, behavioral health, or unmet basic needs. Instead of punishment, PREVAIL connects people to services that address the root problem. The Complex Case Manager helps participants meet basic needs — food, clothing, shelter — and connects them to community resources. This support helps reduce reoffending and helps people build a stable foundation. The Complex Case Manager stays involved from the moment of referral through ongoing case management, meeting participants wherever they're comfortable. All applicants must undergo a pre-employment drug screening, which includes testing for marijuana, as well as a comprehensive background check and TB Test. Staff must pass an extensive background check due to contact with juveniles.

Requirements

  • High school diploma or GED required.
  • Must earn a Certified Addiction Technician (CAT) certification within 18 months, plus a case manager credential.
  • Willing to work a flexible schedule, including days and evenings, and be available on-call.
  • Comfortable working in the field and community — public places, parks, schools.
  • Able to build trust with people facing substance use, behavioral health, or justice involvement — including youth and their families.
  • Strong writing, organization, and computer skills (Microsoft Office).
  • Valid driver's license and clean driving record; able to transport participants.
  • Works with participants 18 and up.
  • Familiar with law enforcement, courts, housing, and social services.
  • Works with participants ages 15 to 17.
  • Must pass an extensive background check.
  • Familiar with municipal courts, schools, child welfare, and youth services.
  • Ability to respectfully engage with people experiencing homelessness, substance use disorder, and mental health disorders.
  • Willingness to follow harm reduction principles.
  • Experience in data collection, case management, and clients struggling with SUD.
  • Experience developing safety plans.
  • Ability to work with diverse populations in various states of anxiety, sobriety, and aggression.
  • Ability to verbally de-escalate individuals.
  • Ability to visually assess for mental or physical health crisis.
  • Ability to visually assess for intoxication.
  • Ability to quickly problem-solve and make decisions.
  • Computer literate with basic knowledge of Microsoft Office Suite.
  • Must possess a valid driver's license and be insurable.

Nice To Haves

  • A degree in behavioral health, human services, social work, criminal justice, education, or a related field is preferred.
  • Familiarity with PREVAIL or similar pre-arrest diversion models.
  • At least two (2) years of experience in the behavioral health or human services field preferred.
  • Experience in case management, diversion, or outreach (adult or juvenile); experience with justice-involved or behavioral health populations.

Responsibilities

  • Accept participants from police custody, diverting them from arrest and charges into services.
  • Partner with law enforcement to connect people to help instead of jail.
  • Serve as the main point of contact between law enforcement, participants, and service providers.
  • Accept social referrals from community partners, in addition to law enforcement.
  • Accept referrals from municipal court, schools, school resource officers, and community stakeholders.
  • Work with the courts during the diversion process — attend hearings and share progress updates with a current signed Release of Information (ROI).
  • Accept social referrals from community partners.
  • Partner with schools and youth organizations to identify young people who could benefit from diversion.
  • Serve as the main point of contact between courts, schools, families, participants, and service providers.
  • Provide basic needs like food, clothing, and shelter.
  • Connect participants to housing, treatment, benefits, and other resources — for youth, this includes helping families and schools too.
  • Help participants stay engaged in services and, for youth, stay in school.
  • Meet participants in the field — public places, parks, or schools for youth.
  • Build trust by meeting people where they are, physically and emotionally.
  • Balance office and community work.
  • Be available for on-call coverage as needed.
  • Work a flexible schedule, including days and evenings, in the office and community.
  • Take on other duties as needed to support the PREVAIL Program.
