About The Position

Case Manager Make a Difference Every Day Are you passionate about helping individuals overcome barriers and achieve meaningful recovery? As a Case Manager, you will play a vital role in supporting adults living with severe mental illness by connecting them to the services, resources, and relationships they need to thrive. This is a community-based position that combines advocacy, care coordination, and relationship-building to empower individuals on their recovery journey. You'll work alongside an interdisciplinary team of behavioral health professionals, medical providers, families, and community partners to ensure every person receives compassionate, person-centered care.

Requirements

  • Bachelor's degree in Social Work, Psychology, Rehabilitation, Human Services, or a related behavioral health field.
  • Equivalent education and relevant experience may be considered in place of a degree.
  • Valid, non-expired driver's license.
  • Reliable transportation with current comprehensive automobile insurance and a satisfactory driving record.
  • Willingness to travel throughout the service area and transport clients as needed.
  • Ability to successfully complete required background screenings, including criminal history and Kansas Child Abuse/Neglect Registry clearance.
  • Ability to perform the essential functions of the position with or without reasonable accommodation.

  • Experience working in behavioral health, community mental health, outreach, or case management.
  • Experience supporting individuals living with severe and persistent mental illness or serious emotional disturbance.
  • Knowledge of recovery-oriented, trauma-informed, and community-based services.

Responsibilities

  • Build trusting, recovery-oriented relationships with individuals and their families.
  • Complete comprehensive assessments and collaborate with clients and the treatment team to develop individualized care plans.
  • Coordinate services across behavioral health, medical, housing, employment, education, and community resources.
  • Advocate for clients with healthcare providers, schools, courts, social service agencies, and other community organizations.
  • Connect individuals to benefits, community resources, and supportive services that promote independence and wellness.
  • Help clients navigate crises by providing support, intervention, and linkage to appropriate services.
  • Monitor client progress and adjust care plans as needs change.
  • Maintain timely, accurate documentation that meets Medicaid, regulatory, and organizational standards.
  • Participate in treatment team meetings, supervision, and ongoing professional development.
  • Travel throughout the community to meet clients where they are, including providing transportation when appropriate.
  • Promote culturally responsive, trauma-informed, and person-centered care in every interaction.
