The Department of Mental Health's Brockton Multi Service Center (BMSC) is seeking an experienced Adult Case Manager to provide assessment, outreach, support, service coordination and person centered advocacy to DMH authorized adults as well as provide open access to support the greater community (inclusive of non-DMH authorized adults) as indicated; promote DMH’s philosophy of recovery, trauma informed care, person-centered choices, and cultural humility by utilizing strength-based goal setting and through independent decision-making; conduct comprehensive assessment and related screenings as well as develop, implement and monitor individual service plans (ISPs); assist individuals in securing services; involve the individual and other identified supports/providers in treatment planning and support recovery goals; work as a member of a larger team; monitor treatment, engagement and responsivity to interventions utilized and is involved in discharge planning for those in acute and long term inpatient care; complete utilization reviews; utilize the electronic and paper record; provide casework and supportive counseling; perform all duties to reflect the highest level of dignity for the persons served. Please Note: This position is 40 hours per week, Work Schedule: 8:30 a.m. to 5:00 p.m. The days off are Saturday and Sunday.
