About The Position

BAYADA Home Health Care is recruiting a compassionate Admissions Registered Nurse to serve patients throughout Montgomery County. The nurse provides skilled, one-to-one care in patients’ homes, supporting them from hospital discharge toward safe and dignified life at home. Care is delivered in accordance with each individual care plan and includes medication administration, wound care, treatments, and comfort care. The role also conducts wound care consultations, provides oversight, assesses patients, educates patients and/or families, and monitors patient conditions.

Requirements

• Active, current license to practice as a professional registered nurse in Pennsylvania.
• At least one year of nursing experience.
• Prior home care experience.
• Ability to travel to assigned cases.
• Calm and reassuring presence, active listening skills, excellent assessment skills, and strong written and verbal communication.
• Ability to deliver patient care with compassion, excellence, and reliability.

Responsibilities

• Support clients through their care needs from hospital discharge to safe home living with comfort, independence, and dignity.
• Deliver one-to-one skilled nursing, interventions, and supportive care in accordance with individual care plans.
• Administer medication and provide wound care, treatments, and comfort care.
• Conduct wound care consultations and provide oversight.
• Perform client assessments, educate patients and/or their families, and monitor patient conditions.
• Travel to assigned cases.
